4. If the nominee is a Philhellene, describe the nominee’s “Greek” connection (for example, describe any relationship to the HHF or Greek community and/or advancement of projects that benefit the Greek-Canadian community or Hellenic ideals).

The Outstanding Achievement Under 40 Award will be presented by the Hellenic Heritage Foundation (HHF) to an outstanding individual that meets the following criteria:

Distinguished Canadian of Hellenic descent, or a Philhellene (from Canada or abroad)
Outstanding leader in the community (this could be global, Canadian and/or Greek-Canadian community)
Exemplary achievement in his or her professional career, industry or field of endeavour
Outstanding contribution to philanthropy and community involvement (measured in terms of significant contributions to charitable causes, organizations, institutions and activities that advance societal goals for a better life, better opportunities, more caring or just society).
Please note that self nominations will not be accepted and only completed nomination forms will be evaluated.

The following information will assist the Selection Committee in assessing the nominee’s eligibility for the award. The form can be completed online below, emailed to [email protected] or mailed to the Hellenic Heritage Foundation, 44 Upjohn Road, Toronto ON M3B 2W1. Nominations should remain confidential. There is no obligation on the part of the nominator to inform the nominee of the nomination.
